Job summary

Florence Home in Omaha, NE is seeking Nursing Care Partners to provide direct patient care in long-term-care, skilled care, and hospice settings, and to collaborate with an interdisciplinary team for comprehensive care.

We require a valid Nebraska LPN or RN license, EMR proficiency, and strong nursing knowledge, with the ability to lift and transfer patients safely and communicate effectively with patients and families.

Qualifications

• Provide direct patient care in long-term-care, skilled care, & hospice settings.
• Collaborate with interdisciplinary healthcare team to ensure comprehensive patient care.
• Educate patients and families on disease management, treatment plans, and self-care techniques.

Responsibilities

• Provide direct patient care in long-term-care, skilled care, & hospice setting.
• Assess patient needs, develop and implement nursing care plans.
• Administer medications and treatments as prescribed by physicians.
• Monitor patient vital signs and report any changes or abnormalities.
• Collaborate with interdisciplinary healthcare team members to ensure comprehensive patient care.
