Qualifications

• Minimum 0 years of experience in a related field.
• High school diploma or equivalent required, with a degree in Nursing (or related field) preferred.
• Current LPN/LVN license in the state of Ohio.
• Ability to lift, move, and transfer patients weighing up to 250 pounds.
• Excellent communication, interpersonal, and problem-solving skills.
• Familiarity with electronic medical records (EMRs) and other healthcare technologies.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and medical treatments as prescribed by physicians.
• Monitor and report patient conditions, vital signs, and any changes to healthcare providers.
• Develop and implement individualized care plans to meet the unique needs of each resident.
• Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal outcomes.
• Maintain accurate and detailed records of patient care, including progress notes and medication administration.
• Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.
