• Monitor vital signs, assess resident status, and report changes to the healthcare team.
• Develop and implement individualized care plans with the interdisciplinary team.
• Collaborate with rehabilitation staff on treatment plans.
• Maintain accurate documentation of resident care and services.
• Assist with admissions, transfers, and discharges as needed.
• Perform other duties as assigned by the RN or Nurse Manager.
