Job summary

ChristianaCare is seeking a Home Health Aide to join the clinical team. The role works under the guidance of a case-managing RN or occupational therapist to provide care to clients per their Plan of Care.

Responsibilities include following the Plan of Care, maintaining infection control, and supporting patient independence while documenting care and reporting observations to the supervising nurse.

Qualifications

• Completion of CNA or Home Health Aide training program (75+ hours).
• 24 months of consecutive work as an aide since program completion is required.
• Ongoing competency evaluations and trainings per agency policy.

Responsibilities

• Follows Plan of Care developed by RN/Therapist.
• Adheres to infection control policies and HIPAA guidelines.
• Provides individualized personal care tasks as outlined in the aide care plan.
• Maintains a safe, clean environment for the patient.
• Encourages independence and goals established by RN/Therapist.
• Reports observations to RN/Therapist relevant to the Plan of Care.
• Monitors and records temperature, pulse, and respiration as per plan.
• Recognizes and reports changes in skin condition.
• Provides additional patient-care skills once deemed competent.
• Respects patient privacy and property.
• Performs safety-related tasks and reports hazards to supervisor.
• Participates in competency assessments and trainings.
