Job summary

Qualifications

• Two years of medical setting experience preferred; not required.

Responsibilities

• Conducts patient assessment interviews and provides referrals.
• Coordinates discharge plans and communicates with care team.
• Maintains patient records and documents critical information.
• Participates in on-call rotation and keeps professional knowledge up to date.
• Adapts services to patient populations and ensures privacy.
