Kern River Transitional Care in Bakersfield, CA is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our team as a full-time Treatment Nurse. The ideal candidate will have strong clinical and documentation skills, attention to detail, and a commitment to high-quality, resident-centered care.

Responsibilities include wound assessments, treatments, accurate documentation, collaboration with the interdisciplinary team, and participation in care planning and quality

Qualifications

• Current California LVN license in good standing.
• Current CPR/BLS certification.
• Wound care or treatment nurse experience preferred.
• Skilled nursing or long-term care experience preferred.
• Knowledge of wound prevention, assessment, treatment, and documentation.

Responsibilities

• Perform wound assessments and treatments per physician orders and care plans.
• Monitor skin condition, wounds, pressure injuries, and treatment needs.
• Document treatments, wound measurements, progress, and changes in condition.
• Notify physicians and nursing leadership of significant findings.
• Ensure records and orders are complete and current.
• Collaborate with nursing staff, physicians, therapists, and the inter-disciplinary team.
• Assist with developing and updating individualized care plans.
• Implement pressure injury prevention and skin integrity programs.
• Educate residents, families, and staff on wound care and prevention.
• Maintain supplies and ensure equipment is clean and stored.
• Follow infection-control practices and facility/state/federal requirements.
• Participate in care conferences and quality assurance activities.
