Diablo Valley Post Acute in Pleasant Hill, CA is hiring a Licensed Vocational Nurse (LVN) for a part-time role with AM & PM shifts. The LVN provides direct nursing care, administers medications, and supervises CNAs to ensure high-quality resident care in a team-oriented setting.

Qualified candidates have an active LVN license and experience in skilled nursing or post-acute care. Strong communication and clinical judgment are required to thrive in a fast-paced environment.

Qualifications

Responsibilities

• Provide direct nursing care and treatments per physician orders and care plans.
• Administer medications and monitor resident responses.
• Observe and report changes in resident condition to nursing leadership and physicians as appropriate.
• Maintain accurate and timely clinical documentation.
• Supervise and support CNAs during assigned shifts.
• Ensure nursing care is delivered safely, effectively, and compassionately.
• Promote a collaborative work environment among nursing staff.
• Follow federal, state, and facility regulations and policies.
