Life Care Home Health is seeking a licensed LVN/LPN to provide compassionate nursing care in patients' homes across our service areas. You will work under the RN, help identify needs, and coordinate care with families and the interdisciplinary team.

Responsibilities include delivering quality care, documenting in the medical record, and participating in ongoing education to maintain competencies. Valid LVN/LPN license and CPR are required, with a valid driver’s license and reliable transport.

Qualifications

• 1 year experience as LVN/LPN in a clinical setting.
• Licensed LVN/LPN in good standing.
• Current CPR certificate required.

Responsabilités

• Promote company mission, values and quality patient care.
• Assist RN in identifying patient needs and care planning.
• Provide direct nursing care during visits to patients' homes.
