Job summary
Md-Home-Health-Md-Home-Assis seeks a Licensed Vocational/Practical Nurse to provide skilled nursing care in patients' homes under the supervision of an RN. You will participate in care coordination with the interdisciplinary team, patient, family, and referring agency.
Candidates must have an accredited LVN program diploma, an active LVN license (NLC allowed), a valid driver's license, and at least 1 year of LVN experience in a clinical setting; home health experience is preferred.
Qualifications
- Graduate of an accredited school of vocational/practical nursing.
- Active state LVN license (or NLC) and valid driver's license.
- At least 1 year of LVN experience in a clinical setting; home health preferred.
Responsibilities
- Administer skilled nursing care under RN supervision.
- Coordinate care with the interdisciplinary team, patient, family and referring agency.
- Travel to patients' homes and work in varied conditions.
