AccentCare, Inc. is seeking an LVN/LPN for field-based home health nursing in the Redding, CA area. Under RN supervision, you’ll provide direct patient care to pediatric through geriatric clients and document progress and nursing status changes.

Weekdays 8 am–5 pm, with travel to designated locations. Must graduate from an accredited LVN/LPN program, hold a current license in CA, have CPR certification, and at least one year of LVN/LPN experience. Valid driver's license required.

Qualificações

• Graduate of an accredited practical or vocational nursing program.
• Current LPN/LVN license in the operating state.
• Minimum one year LVN/LPN work experience.

Responsabilidades

• Provide direct patient care under RN supervision.
• Document patient progress and nursing status changes.
• Administer prescribed medications and treatments as allowed by law.
• Re-evaluate nursing needs and report findings to Case Manager.
• Counsel and educate patient and family on nursing needs.
• Prepare clinical and progress notes and ensure timely documentation.
• Coordinate services with home health team.

Conhecimentos

LVN/LPN experience
