AccentCare, Inc. in Petaluma is seeking a part-time LVN/LPN for Home Health. Under the supervision of an RN, you will provide direct patient care and document nursing progress for pediatric through geriatric patients.

Schedule is Monday–Friday, 8 am–5 pm, with a coverage area including Petaluma, Sonoma and surrounding Marin County. Requires graduation from an accredited practical nursing program, current LVN/LPN license in the state of operation, CPR certification, and at least one year of

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Currently Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation.
• One year of LVN/LPN work experience.

Responsabilités

• Performs technical nursing functions and direct patient care under RN supervision.
• Promotes and maintains optimal client care under RN supervision.
• Maintains a safe patient environment.
• Identifies and communicates significant abnormalities to the case manager and home health team.
• Administers prescribed medications and treatments, and reviews medications for potential interactions.
• Re-evaluates client’s nursing needs and reports findings to the Case Manager.
• Counsels and instructs the client and family in meeting nursing needs.
• Prepares clinical/progress notes and submits documentation timely.
• Ensures chart documentation aligns with the nursing care plan.
• Communicates with the home health team and coordinates services.
