Job summary

Atlas Home Health in Napa County is seeking a Licensed Vocational Nurse (LVN) to provide continuous, one-to-one skilled nursing care in the patient’s home, typically through 8-hour shifts, under the supervision of a Registered Nurse.

This part-time role focuses on direct patient care, implementing established plans of care, and coordinating with the clinical team while following CMS/Title 22 standards. CA LVN license and CPR are required; driving is expected with a reliable vehicle.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Currently licensed as an LPN/LVN in California.
• CPR certification required.
• 1 year nursing experience preferred for independent field assignment; home health/acute care preferred.
• Valid driver’s license and a reliable vehicle.
• Strong verbal/written communication and organization skills.

Responsibilities

• Provide direct patient care as defined in the California Nurse Practice Act.
• Implement and revise the plan of care for patient status changes.
• Deliver skilled nursing care, including medications and treatments as prescribed.
• Prepare accurate clinical notes and coordinate with the RN/Clinical Supervisor.
• Maintain equipment, adhere to aseptic techniques, and educate patients and families.

Education

Licensed Practical Nurse (LPN) / Vocational Nurse (LVN) license (California)

CPR certification

1 year nursing experience (preferred)
