Job summary

Qualifications

• Associate degree in PTA from CAPTE‑accredited program.
• Current unencumbered Ohio PTA license required.
• One year of hospital, long-term care, or home health experience preferred.

Responsibilities

• Implements Plan of Care per Physical Therapist’s orders.
• Communicates with PT, physician, and staff about patient concerns.
• Collaborates on patient assessments (OASIS).
• Educates patients and families on exercises and self‑care.
• Participates in plan of care meetings and discharge planning.
• Conducts home visits to apply the Plan of Care.
