Job summary

Ohio Living is seeking a Licensed Physical Therapy Assistant to join our Home Health & Hospice team. You will implement the Plan of Care set by the Physical Therapist, perform treatments at patients' homes, and ensure care follows physician orders to maintain function and safety.

You will educate patients and families on exercises and self-care, collaborate with the clinical team, and participate in discharge planning, while keeping records up to date in the EMR and maintaining a clean, safe

Qualifications

• Associate degree in Physical Therapy Assistant Technology from CAPTE or equivalent.
• Current Ohio LPTA license to practice.
• Must read, write, speak and understand English.
• Valid driver’s license and insurable under policies.

Responsibilities

• Implement Plan of Care developed by the Physical Therapist and follow physician orders.
• Communicate patient status with PT, physician and staff to update the Plan of Care.
• Educate patient and family on exercises, precautions, and self-care for safety.
• Participate in interdisciplinary team meetings and discharge planning.
• Conduct home visits to guide self-care and improve function.
• Deliver treatments in line with productivity standards.
• Maintain clean, safe treatment areas and equipment.
• Maintain accurate patient records in the EMR according to standards.

Essential Activities and Tasks

Expectations and Care Management - 70%

Implements the Plan of Care developed by the Physical Therapist ensuring that all patients are treated per the physician’s orders. Communicates with the Physical Therapist, physician, and other pertinent staff about physical/medical concerns pertaining to the Plan of Care in order for appropriate revision, as the patient’s condition requires. Collaborates with clinical team and Physical Therapist in the implementation of the patient assessments (OASIS). Educates and instructs patient, family members and other health team members on the proper techniques for an exercise program, precautions and self care activities to assure safety for caregivers and patients. Participates in the interdisciplinary team meeting by exchanging information on progress and discharge plan in order to provide the highest level of continuity of care for the patient. Participates in the plan of care meetings to communicate to the patient, family members and patient advocates the patients’ progress and discharge plan. Conducts home visits to implement the patient’s Plan of Care, to guide the patient in the use of therapeutic and self-care activities for the purpose of improving function and safety in order to allow the patient to remain at home at their highest level of function and independence. Ensures that all work areas, treatment areas and equipment are maintained in a clean, sanitary and safe manner. Organizes and performs the delivery of physical therapy treatments in accordance with company productivity standards.

Administration and Reporting - 20%

Maintains current and accurate patient records through the use of electronic medical record documentation, in accordance with company standards and federal, state, and local standards, guidelines, and regulations.

Quality, Compliance, and Risk Management - 10%

Maintains compliance with all standards, regulations and rules pertinent to services provided. Maintains the confidentiality of patient information and honors the patient’s personal and property rights. All other duties as assigned.

Qualifications Education

Associate degree in Physical Therapy Assistant Technology from a Commission on Accreditation in Physical Therapy Education (CAPTE) or equivalent accredited program required. Current unencumbered license for the state of Ohio to practice as a Physical Therapy Assistant (LPTA) required.

Experience

One year experience as a Licensed Physical Therapy Assistant in a hospital, long-term care facility, or home health agency preferred. New graduates welcome. Experience in the care of geriatric and physical therapy patients required. Knowledge of treatment modalities.

Other Requirements

Must be able to read, write, speak, and understand the English language. Must possess a valid driver’s license and acceptable driving record. Must be insurable under professional liability and crime coverage policies as specified by insurance carrier underwriting standards.
