Job summary

Emerald Care Center Southwest in Oklahoma City is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care, assess resident needs, and participate in care planning with the healthcare team.

You will work in a skilled nursing facility, monitor vital signs, administer medications, document care, and ensure compliance with infection control and safety protocols. Strong communication and teamwork are essential.

Qualifications

• Valid LPN/LVN license in Oklahoma required.
• Graduate from an accredited LPN/LVN program.
• CPR/BLS certification current; additional state requirements may apply.
• Excellent verbal and written communication; team-oriented.
• Physical ability to lift/move patients and stand long periods.

Responsibilities

• Provide direct nursing care, including medications, wound care, and treatments as prescribed.
• Monitor vital signs, document progress, and report changes to charge nurse/ DON.
• Assist in individualized care plan development and implementation.
• Maintain accurate care documentation and effective communication with residents/families.
• Adhere to infection control, safety protocols, and facility policies.
• Offer emotional support and education to residents and families; collaborate with the care team.
