Job summary

Qualifications

• Valid LPN license in the State of New York
• Experience in Skilled Nursing preferred but not required

Responsibilities

• Order prescribed medications, supplies, and equipment as needed to support resident care
• Follow individualized care plans tailored to each resident's unique needs
• Perform assigned duties including wound care, treatments, and clinical procedures with precision
• Accurately document all observations, interventions, and resident evaluations
• Maintain strict confidentiality and uphold residents', families', and facility's rights in accordance with HIPAA regulations
