Job summary

Trustwell Living at Carlisle Place in Bucyrus, OH is seeking a Licensed Practical Nurse (LPN) to provide direct resident care and supervise residents’ needs according to their individualized care plan. Qualifications

• LPN license valid and unencumbered; compact license encouraged.
• Minimum of 1 year as LPN in assisted living/long-term care.
• Experience administering medication per doctor's orders and community policies.
• Knowledge of state regulations for resident care.
• Current driver’s license.

Responsibilities

• Provide direct resident care based on individualized care plan.
• Supervise residents’ needs and assist in supervising the care team.
• Monitor residents for changes in condition and signs of illness.
• Communicate and document changes or concerns with Health Services Director.
