Job summary

Trustwell Living at Bell Gardens Place is seeking a Licensed Practical Nurse (LPN) to provide direct resident care and supervise the care team according to individualized care plans. You will monitor residents for changes in condition and document findings in accordance with laws and community policies.

A compact license or the ability to obtain one is encouraged, but an unencumbered LPN license is required.

Qualifications

• LPN license valid in the state.
• Minimum 1 year of LPN experience in assisted living or long-term care.
• Experience administering medications per orders and policies.
• Knowledge of state regulations pertaining to resident care.
• Experience as a mandatory reporter and ability to investigate concerns.
• Valid driver’s license.

Responsibilities

• Provide direct resident care and supervision based on individualized plans.
• Monitor residents for changes in condition and document findings.
• Communicate and document concerns with the Health Services Director and supervise the care team.
