The role supports ADLs and resident well-being in a compassionate, team-focused environment. Current TN LPN license, CPR certification, and the ability to pass a background check are required.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and treatments.
• Monitor vital signs, report changes to the healthcare team, and take appropriate action.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Maintain accurate and detailed records of patient care and medication administration.
• Participate in Quality Assurance Program and adhere to policies and procedures.
• Collaborate with other healthcare professionals to ensure seamless and efficient care.
• Assist with activities of daily living (ADLs) and contribute to residents' well-being.
