Qualifications

• Must have current TB skin test or Chest X-ray.
• Valid driver's license if assigned to driving cases.
• Previous experience preferred but not required.

Responsibilities

• Provide medical and personal care based on individual Plan of Care.
• Orient new caregivers to clients' plan of care.
• Communicate changes in client medical status to Nurse Supervisor.
• Escort clients to appointments utilizing client vehicle.
• Confirm weekly schedules with the Staffing Coordinator.
• Ensure timely required documentation for compliance.
