Qualifications

• Must have a valid driver’s license if assigned to a case with driving privileges.
• Current TB skin test or Chest X-ray required.

Responsibilities

• Provide medical and personal care based on the individualized Plan of Care.
• Orient new caregivers to clients' Plan of Care.
• Communicate changes in client medical status to Nurse Supervisor.
• Drive clients to and from appointments as needed.
• Document required information to ensure compliance.
