Job summary

St Johnsbury Center for Living and Rehabilitation in Vermont is seeking a compassionate LPN to join our team. You will provide direct patient care, administer medications, perform wound care, monitor vital signs, and keep accurate records to support individualized care plans.

You will collaborate with physicians, nurses, and other clinicians during rounds, participate in interprofessional teamwork, and uphold safety, dignity, and respect for residents.

Qualifications

• Current LPN license in Vermont is required.
• High school diploma or equivalent; an associate's or bachelor's degree in nursing is preferred.
• Strong communication and interpersonal skills; ability to work in a fast-paced environment.

Responsibilities

• Provide direct patient care including administering medications and wound care.
• Develop and implement individualized care plans with other healthcare professionals.
• Conduct initial patient assessments and maintain accurate records.
• Participate in interprofessional rounds and communicate with patients, families, and colleagues.
• Uphold high standards of patient safety, dignity and respect.
• Perform other duties as assigned by nursing leadership.
