Job summary

Affinity Hospice Care Inc. is seeking an experienced Licensed Practical/Vocational Nurse to provide direct patient care in patients' homes in California. The role emphasizes teamwork, compassionate support, and adherence to care plans established by RNs.

Per diem positions offer flexibility with a practical travel schedule, a competitive pay per visit, and opportunities for skills development and potential full-time conversion based on performance.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
