Affinity Hospice Care Inc. is seeking a per diem Licensed Practical/Vocational Nurse to provide direct patient care in patients’ homes across California.

You will implement care plans, educate families, document details, and collaborate with the RN and care team to ensure safe, compassionate home hospice services. The role requires a CA LPN/LVN license, CPR certification, and 3+ years of nursing experience.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.

Responsabilités

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Connaissances

CPR certification (AHA)

LPN/LVN license

Strong communication

Formation
