Encompass Health is seeking an LPN/LVN to provide compassionate, patient-centered care within our rehabilitation setting. You will work closely with the care team to follow physician orders, monitor patient progress, and support meaningful recovery for individuals facing rehabilitation needs.

Qualifications

• Current LPN/LVN licensure required by state regulations.
• CPR certification required.
• One year of inpatient medical-surgical or general hospital experience preferred.
• Rehabilitation experience preferred.

Responsibilities

• Provide direct patient care aligned with the care plan and physician orders.
• Monitor vital signs, symptoms, and treatment responses.
• Collaborate closely with Registered Nurses to report findings and execute care plans.
• Build meaningful relationships with patients to support their physical, mental, and emotional recovery.
• Celebrate patient victories along the recovery journey.
