Signature HealthCARE is seeking a licensed practical nurse to provide direct nursing care, assist residents with daily activities, and supervise staff. The role emphasizes documentation, care planning, and adherence to long-term care regulations.

You will work in a supportive team, coordinate admissions and discharges, and participate in quality assurance activities to improve resident outcomes.

Qualifications

• Active LPN/LVN license in the state of practice.
• Experience in a supervisory or charge nurse role.
• Familiar with EHR systems and standard office tools.

Responsibilities

• Provide direct nursing care to residents, including meds and wound care.
• Assist with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing staff and coordinate shift activities.
• Monitor resident status and notify physicians of changes.
• Develop and review individualized care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA, education, and regulatory surveys.
