Princeton Transitional Care & Assisted Living, part of Signature HealthCARE, seeks a caring LPN/LVN to provide direct nursing care in a skilled nursing setting in Erwin, TN. You will supervise CNAs, manage medications, document thoroughly, and participate in care planning to support residents’ quality of life.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents' condition and notify physician of changes.
• Develop and review individualized resident care plans with the team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in QA activities, staff education and regulatory surveys.
