Job summary

Reliant Care Management Company seeks a licensed practical nurse (LPN) to provide compassionate resident care in our facility in Kentucky. You will perform assessments, wound care, medication processing, and coordinate with physicians and guardians as needed.

The role leads CNAs and CMTs, ensures adherence to protocols, and responds promptly to emergencies. A current LPN license and commitment to patient-centered care are required, with competitive benefits included.

Qualifications

• LPN licensure with active status.
• Proven patient care and wound care experience.
• Ability to assess, document, and report changes promptly.
• Experience leading CNAs/CMTs preferred.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart setup, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
