Altra Home Health is seeking a dedicated Licensed Practical Nurse (LPN) to provide in-home care to pediatric and adult patients in Peru, IN. You will deliver personalized nursing care, manage devices like G-tubes, and assist families in disease management within the comfort of the patient’s home.

Qualifikationen

• Active LPN license in Indiana or compact LPN license.
• Current CPR certification.
• 1–2 years clinical nursing experience (home health/pediatrics preferred).

Aufgaben

• Conduct head-to-toe assessments and implement care plans.
• Deliver hands-on care: G-tube feedings, medications, wound care, trach care, vent support.
• Educate patients and caregivers on disease management and emergency response.
• Collaborate with physicians, therapists and team members to deliver quality care.
• Document care activities accurately per agency and state standards.
• Maintain safety, infection control and patient privacy.
