Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Valid driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws.
• Documents patient visits thoroughly in the electronic medical record according to regulatory and payer requirements.
• Participates in care coordination, communicates with patient, family, physician, and care team.
• Functions in any home setting regardless of client demographics and conditions.
