Aegis Home Health and Hospice is hiring a Full-Time LPN to join our Omaha-based home health team. You will perform skilled nursing visits, assess patients holistically, coordinate care with a multidisciplinary team, and maintain precise patient records.

You’ll liaise with families to ensure smooth plan implementation and timely referrals as needed. The role emphasizes compassionate caregiving, autonomy, and professional collaboration, with ongoing training and a strong focus on patient-centered

Qualifications

• LPN license: Nebraska (Iowa license a plus).
• Home Health/Hospice experience preferred.
• Driver's license and reliable transportation required.
• CPR certification and TB test current.

Responsabilités

• Administer prescribed treatments and medications.
• Support ADLs and pain management.
• Teach patients and caregivers practical self-care techniques.
• Observe patient status and report changes to RN/physician.
• Participate in interdisciplinary care planning discussions.
• Keep documentation accurate and ensure supplies are on hand.
