Millennium Physician Group in Naples, FL seeks a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) to join our Home Health Team. You will provide skilled nursing care under the supervision of a Registered Nurse and contribute to curative, rehabilitative, and preventive care plans.

Qualifications

• Graduate of an accredited School of Practical Nursing.
• Current, unrestricted LPN license in the state.
• CPR/BLS certification or ability to obtain within required timeframe.
• Experience documenting patient care in an EMR.
• Knowledge of nursing principles and infection prevention standards.
• Ability to work independently with RN collaboration.

Responsibilities

• Provide skilled nursing care per physician orders and plan of care.
• Collaborate with RNs to implement and update patient care plans.
• Observe and report changes in patients’ health status.
• Assist with treatments, medications, and procedures as allowed.
• Educate patients and caregivers on disease management and self-care.
• Document assessments and progress in the EMR within required timeframes.
• Communicate with patients, families, and care teams to coordinate care.
• Participate in team meetings and ongoing professional development.
• Maintain compliance with policies and infection prevention protocols.
• For qualified advanced LPNs, perform approved IV therapies per regulations.
