Job summary

Elara Caring is hiring a Licensed Vocational/Practical Nurse to join our home health care team. You will follow individualized care plans, monitor patient progress, and educate patients and families on self-care techniques to support overall health.

Work collaboratively with the RN Case Manager to adjust care plans as patient needs evolve, document medications, and communicate with physicians and families to ensure coordinated, safe care in the home setting.

Qualifications

• Current State License as a Licensed Vocational Nurse or Practical Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, or ADN).
• Minimum 1 year of clinical care experience.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes to the care team.
• Assist patients and families with self-care techniques and education.
• Collaborate with the RN Case Manager to update care plans as needs change.
• Document medication regimens and update medication profiles per guidelines.
• Communicate with physicians, patients, families and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations through proactive care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
