Job summary

Lakeview Village in Lenexa, KS is hiring a Licensed Practical Nurse (LPN) for Home Health. You will provide skilled nursing care under the direction of an RN Case Manager, deliver patient education, and ensure safety and dignity in the home setting.

Responsibilities include following the plan of care, documenting patient status, administering medications, coordinating with the RN Case Manager, and participating in on-call coverage as needed.

Qualifications

• Must have current, unrestricted LPN license in Kansas.
• Current CPR certification required.
• Home health experience preferred.

Responsibilities

• Provide skilled nursing care within LPN scope per plan of care.
• Document information and report changes to RN Case Manager.
• Administer medications and monitor treatment responses.
• Educate patients and caregivers on medications and disease management.
• Coordinate with RN Case Manager and healthcare team.
• Participate in weekend and on-call coverage as needed.
