Full-Time | Lakeview Village

Provides skilled nursing services to patients and their families in the home setting under the direction and supervision of an RN Case Manager or the Director of Nursing. Promotes health, supports recovery, and helps patients maintain their highest possible level of independence. Serves as a patient advocate while delivering compassionate, coordinated care.

Essential Duties and Responsibilities
  • Provides skilled nursing care within the LPN scope of practice, following physician or allowed practitioner orders and the established plan of care.
  • Collects and documents information regarding patients’ physical, emotional, and mental health needs and promptly reports changes in condition to the RN Case Manager and appropriate provider.
  • Contributes to the development and ongoing review of the plan of care by communicating patient needs, treatment responses, and progress toward goals.
  • Performs ordered nursing treatments, including wound care, catheter care, and medication administration, within demonstrated competency and agency policies.
  • Monitors medications and reports discrepancies, side effects, and concerns to the RN Case Manager and appropriate provider.
  • Completes accurate and timely visit documentation, including care provided, patient responses, education, and communication with the healthcare team.
  • Provides and reinforces education to patients and caregivers regarding medications, disease management, treatments, safety, and self-care.
  • Collaborates with the RN Case Manager, therapists, Home Health Aides, and other team members to promote continuity of care.
  • Communicates observations or concerns regarding Home Health Aide services to the RN Case Manager.
  • Participates in patient care conferences as needed.
  • Follows standard precautions, infection prevention procedures, and safety standards.
  • Helps coordinate needed medical supplies, equipment, and services with the RN Case Manager and office staff.
  • Observes patients’ living environments, identifies safety concerns, and reports needs that may affect care.
  • Coordinates visit times with patients, caregivers, and the care team and promptly communicates scheduling changes or missed visits.
  • Maintains patient confidentiality and respects patient rights, preferences, and dignity.
  • Participates in assigned weekend and on-call coverage within the LPN role, with RN support and supervision available.
  • Maintains required competencies and participates in agency education and quality improvement activities.
Qualifications
  • Must possess a current, unrestricted LPN license authorizing practice in the State of Kansas.
  • Must have current CPR certification.
  • Prior home health experience is preferred.
  • Must have reliable transportation and a valid driver’s license.
  • Must demonstrate strong communication, organizational, and documentation skills.
  • Must be able to manage an assigned visit schedule while working under RN supervision.
  • Ability to communicate effectively in English, including reading, speaking, and writing, when required to perform job duties, and to work effectively with individuals from diverse backgrounds.
