Job summary
Pinnacle Home Care, Florida's largest independent Medicare-certified home health provider, is seeking a Full-time Licensed Practical Nurse (LPN) to administer skilled nursing care in patients' homes across South Naples and surrounding areas.
The ideal candidate has an active LPN/LVN license, graduation from an accredited nursing program, a valid BLS certification, and at least one year of nursing experience in home health or medical/surgical settings.
Qualifications
- Must hold a current LPN or LVN license (state-specific).
- Graduate of an accredited nursing program.
- Valid BLS certification.
- Minimum one year of nursing experience (home health or medical/surgical preferred).
- Excellent communication, organizational, and time-management skills.
Responsibilities
- Administer skilled nursing care under RN supervision.
- Perform curative, rehabilitative, or preventive care per patient plans.
- Use clinical expertise to support patient care and positive outcomes.
- Complete visit reports promptly in EMR, ensuring documentation compliance.
- Coordinate daily visit schedules with other staff.
- Participate in the agency's QAPI program as required.
- Adhere to policies for patient rights and discharge processes.
