Job summary

Wellstar Health System is seeking an Licensed Practical Nurse (LPN) to provide direct patient care in a medical/surgical home health setting. The LPN will follow a defined scope of practice under RN supervision, coordinate care with the team, and participate in patient and caregiver education to support high-quality outcomes.

The role requires a High School Diploma or GED and LPN licensure with BLS certification.

Qualifications

• Requires LPN licensure and BLS certification.
• Experience in medical/surgical nursing preferred.
• Customer-service and patient-education skills a plus.

Responsibilities

• Direct patient care under RN oversight and physician orders.
• Coordinate care with RN and other team members.
• Educate patients and caregivers on care plans and home care.
• Accurately document care and regulatory compliance.
• Participate in staff education and ongoing learning.
• Perform other duties as assigned by Home Health leadership.
