Job summary

Wellstar Health System is seeking a licensed practical nurse (LPN) for home health care in the United States. The LPN works under the direction of a Registered Nurse or Manager to provide direct patient care for medical/surgical patients in the home setting.

The role requires coordination with the care team, patient education, and adherence to regulatory guidelines. A minimum of 1 year of acute care nursing is preferred and BLS certification is required.

Qualifications

• High School Diploma or GED required.
• LPN license and active Basic Life Support (BLS) certification.
• Minimum 1 year of medical/surgical nursing experience in an acute care setting preferred.
• Strong communication and organizational skills; ability to educate patients and families.

Responsibilities

• Provide direct patient care under RN supervision in home health settings.
• Coordinate care with RN and other team members to develop and adjust plans of care.
• Document patient information accurately within required timeframes; adhere to regulations.
• Participate in staff meetings, continuing education, and safety initiatives.
