Job summary

CareRite Centers is seeking a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehabilitation facility. The LPN delivers bedside nursing, administers medications and treatments, monitors resident conditions, and documents in EMRs.

You’ll collaborate with an interdisciplinary team to implement individualized care plans, support rehab and long-term care goals, and educate residents and families.

Qualifications

• Current LPN license in the United States.
• Experience in skilled nursing or rehab preferred.
• Ability to document accurately in EMRs.

Responsibilities

• Provide bedside nursing care to residents.
• Administer medications and treatments as ordered.
• Monitor resident conditions and respond to changes.
• Document assessments, care plans, and progress in EMRs.
• Collaborate with an interdisciplinary team to implement care plans.
• Support rehabilitation and long-term care goals for residents.
• Educate residents and families about care plans and treatments.
