Job summary

Heritage at Brandon Place in Oklahoma City seeks a licensed Practical Nurse (LPN) to provide direct nursing care to residents in a skilled nursing facility under supervision of the RN. You will administer medications, monitor health, assist with care plans, and ensure high-quality care in alignment with facility policies and state regulations.

The role requires an active LPN/LVN license, graduate of an accredited program, CPR certification, and preferred experience in long-term care.

Qualifications

• Current Licensed Practical Nurse (LPN) / LVN license in the state of employment.
• Graduate of an accredited practical nursing program.
• Experience in long-term care or skilled nursing preferred.
• Current CPR certification required.

Responsibilities

• Provide direct nursing care to residents in accordance with care plans and physician orders.
• Administer medications and treatments as prescribed.
• Monitor residents’ vital signs, conditions, and responses to treatment.
• Report changes in resident condition to the RN or physician.
• Assist with development and implementation of individualized care plans.
• Supervise and support CNAs.
