Qualifications

• LPN license must be active in the state.
• CPR certification required to practice.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies, and center policies.
• Collaborate with interdisciplinary team for optimal outcomes.
• Report abnormal changes in residents’ condition per policy.
• Supervise nursing staff in assigned situations.
• Deliver nursing care for residents requiring long-term or rehabilitative care.
• Collect data, observe and report on patient/resident condition.
