Job summary

Trilogy Home Healthcare is hiring a Licensed Practical Nurse (LPN) for Home Health in Pasco County, FL. This PRN/Per Diem role requires weekend coverage and involves patient-focused care under a plan of care.

You will assess, coordinate with clinicians, advocate for patients, and contribute to outcomes that improve daily living. A Florida LPN license and valid driver’s license with insurance are required.

Qualifications

• Graduate of an accredited nursing program.
• Active Florida LPN license.
• Minimum 1 year nursing experience; Home Health preferred.
• IV certification preferred.
• Valid Florida driver’s license with active vehicle insurance and clean MVR.

Responsibilities

• Provide high-quality, compassionate care per the patient’s plan of care.
• Use assessment tools to manage medical needs and coordinate with other disciplines.
• Serve as patient advocate, communicating with physicians, clinical managers, and RNs.
• Make a meaningful impact on patients’ lives every day.
