Responsibilities

• Provide direct and indirect care according to each resident's plan of care.
• Assess, observe, and document residents' clinical condition in the EMR; notify RN/provider as needed.
• Update or modify care plans to reflect changes in resident needs.
• Administer medications timely and accurately per orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents and families in a timely and professional manner.
