Qualifications

• Current LPN license in the state of New Jersey.
• Strong clinical and communication skills.
• Compassionate, reliable, and team-oriented.

Responsibilities

• Assist CNAs with ADLs and routine resident care.
• Administer medications and treatments as prescribed.
• Document and maintain patient records accurately.
• Observe and report resident health conditions to RNs, supervisors, and physicians.
• Monitor vital signs and intake/output.
• Provide wound care, injections, and IV management.
• Support admissions and communicate with the pharmacy for orders.
• Ensure resident safety by reporting incidents or skin breakdowns.
