Job summary

Elderwood at North Creek is seeking a dedicated LPN to join our caring team in North Creek, NY. The LPN will manage daily clinical regimens for residents in a home-like setting, working under the supervision of nursing leadership and collaborating with a multidisciplinary team.

State LPN license required; geriatric experience preferred. We offer shift options, competitive pay, and a comprehensive benefits package to support staff and residents alike.

Qualifications

• LPN or RN with a current state license required.
• LPN training and geriatric care experience preferred.
• Ability to supervise and collaborate with team members required.
• Good written and spoken English communication necessary.

Responsibilities

• Administer medications and treatments per physician orders.
• Direct and monitor nursing care provided by Nursing Assistants.
• Assist with direct resident care as needed.
• Report changes in resident condition to supervisors.
• Follow Plan of Care and monitor care plan implementation.
• Manage medication storage, distribution, and records.
