Job summary

SRVS is seeking a Licensed Practical Nurse (LPN) to provide comprehensive nursing care in SRVS Intermediate Care Facilities in Memphis, TN. You will oversee health needs, medication management, and health monitoring for program participants, coordinating with a multidisciplinary team and families.

The role emphasizes patient safety, regulatory compliance, and accurate medical documentation, with shifts including weekdays evenings and weekend coverage.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Licensed Practical Nurse (LPN) license in good standing.
• Minimum one year of nursing experience preferred.
• Experience working with individuals with IDD or in long-term care, residential, behavioral health, or healthcare settings preferred.

Responsibilities

• Provide direct nursing care and health-related support.
• Administer medications and treatments as prescribed by physicians and authorized healthcare providers.
• Monitor, assess, and document changes in individuals' health conditions and communicate concerns promptly.
• Coordinate medical appointments, treatments, and follow-up care.
• Maintain accurate and timely medical records in accordance with regulatory and organizational requirements.
• Review, update, and implement health care plans as needed.
• Collaborate with physicians, specialists, pharmacies, therapists, families, and interdisciplinary team members.
