Job summary

Advocate Health is seeking a Practical Nurse (LPN) to provide direct patient care under supervision. The role involves assessing patients, documenting findings, and contributing to care plans in accordance with established policies and procedures.

The ideal candidate will hold an approved Practical Nursing program credential, an active LPN license, and BLS certification within 6 months if required by the department.

Qualifications

• Completion of an accredited Practical Nursing program.
• Licensed Practical Nurse (LPN) license for the state of practice.
• Basic Life Support (BLS) for Healthcare Providers certification within 6 months if required by department.

Responsibilities

• Provides direct patient care according to policies, procedures and standards.
• Assists with assessment by collecting and documenting data and reports significant changes to the RN.
• Assists with developing and implementing the plan of care as directed by the RN.
• Supports and reinforces patient teaching.
• Documents nursing interventions and patient responses; communicates changes to the RN.
• Performs clinical tasks such as medication administration, IV therapy, wound care, venipuncture.
• Handles patient positioning and transfer with proper technique.
• Demonstrates age-appropriate care and growth/development awareness.
