Job summary

Avenues Recovery Center in Pennsylvania seeks a dedicated LPN for day shift at Lake Ariel, a 60-bed residential facility offering detox and inpatient rehab. You will provide direct patient care, coordinate medication needs, and work with UR teams to ensure comprehensive treatment planning.

Must be a graduate of an accredited LPN program with PA licensure or compact privileges, strong communication, and the ability to multitask.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Pennsylvania licensure or compact licensure with multistate privileges.
• Strong communication and organizational skills.
• Ability to multitask in a fast-paced environment.
• Compassionate, professional, and committed to providing exceptional patient care.
• Experience in behavioral health or substance use treatment preferred.

Responsibilities

• Provide quality medical care for each client throughout admission, treatment and discharge.
• Coordinate all pharmacy and prescription needs, dispense medication, and keep clear med logs.
• Collaborate with extended medical, clinical and UR teams to ensure total coordination of care.
• Maintain meticulous medical documentation, tracking and reporting for each client.
• Provide patient education in individual and group settings.
