Job summary

Chapters Health System is seeking an LPN for Continuous Care to support patients and families under RN supervision. You will perform nursing tasks, provide comfort measures, and contribute to quality of life within the care plan.

The role requires an LPN license, basic life support, and a flexible schedule to meet patient needs. Mileage and tuition reimbursement are offered, with a 7p-7a shift pattern across Florida affiliates.

Qualifications

• High school diploma or GED required.
• Current LPN licensure in the state of employment.
• 6 months experience as LPN preferred (not mandatory).
• Active BLS for Healthcare Professionals from AHA or Red Cross.
• For current Hope/Hospice employees: BLS by 2025/2024 respectively.
• Mobile Driver – valid license and insurance per company policy.

Responsibilities

• Under RN supervision, provide nursing interventions and teach skills to maximize patient comfort and quality of life.
• Represent the company professionally in care delivery and services.
• Observe and report changes in patient condition to RN.
• Document care in the medical record per policy.
• Perform venipuncture and CVL-related tasks as authorized.
• May be asked to cross-train to Hospice House roles during census fluctuations.
