Qualifications

• Master's degree in social work (MSW) or Licensed Clinical Social Worker (LCSW) preferred.
• Current CPR certification.
• Must have a valid driver's license and a reliable automobile.
• Minimum 1 year of healthcare experience; home health, hospice, geriatrics or end-of-life care preferred.

Responsibilities

• Assess the psychosocial status of patients and families in the home and communicate findings to the interdisciplinary team.
• Provide crisis intervention as needed and support advanced directives.
• Educate the team on cultural needs and family dynamics and plan interventions.
• Maintain accurate clinical records and progress notes.
• Assist in care planning and participate in interdisciplinary meetings.
• Serve as liaison with patients, families, and community agencies.
• Participate in on-call rotation including weekends and holidays.
